Insurance

We are pleased to announce that we are participating providers with the following insurances:

We participate with the following Medical Insurance Companies:

  • Aetna HMO/PPO
  • Anthem HealthKeepers
  • Anthem and Blue Cross Blue Shield PPO (for all states)
  • Anthem FEP

(Please note that we do not participate with Medicaid through any of these medical plans listed above.)

The following is a list of Dental Insurance Companies that we participate with:

  • Aetna PPO
  • Anthem Dental
  • Cigna Dental
  • Delta Dental (PPO) for all states
  • GEHA (but we do not participate with Connection Dental plans through GEHA)
  • Humana Dental
  • Metlife Dental
  • United Healthcare Dental
  • WellPoint Dental

If we do not participate with your insurance plan, we will gladly still file a claim to your insurance for you. However, you remain financially responsible for all charges incurred.

Note: We do not file claims with liability/accident insurance carriers.

Workers Compensation: Written authorization must be obtained from your employer before treating a Workers Compensation injury. Claims for work related injuries are sent to the insurance company or your employer. If not covered by insurance, you agree to be responsible for the balance due for the services rendered.

Medicare & Medicare Advantage Patients:

Commonwealth Oral & Facial Surgery will no longer participate in Medicare or Medicare Advantage plans effective April 1st, 2024. As a Medicare or Medicare Advantage plan beneficiary, you may continue to receive your care from our practice as described below, or you may want to transfer to another provider.

While we prefer to continue to provide you with the exceptional care you have come to expect from us, we understand that your healthcare decisions must be made according to your unique needs and circumstances.

In the past, you have generally only been responsible for charges that were not covered by Medicare or your Medicare Advantage plan. If you continue to receive your care at our practice, you must accept full responsibility for payment for all services furnished at Commonwealth Oral & Facial Surgery. If you arrive at our office for treatment after April 1st, 2024, you will be required to sign a “Private Contract- Provider Opt-Out of Medicare” form. Please ask our front desk for a copy of the contract if you wish to review it.
